The 3d Preoperative Anatomy of the Hip in Total Hip Arthroplasty for Dysplasic Development of the Hip. a Radiographic and Ct-scan Analysis of 312 Cases

In my series of total hip replacement, the cemented Ceraver Osteal endoprostheses was used since 1977 in 150 cases, the titanium uncemented screw cup ring with a ceramic core since 1985 in 131 cases and since 1996 a pressfit titanium cup with a pure titanium fibre mesh with ceramic core in 30 cases. The ceramic head was fixed to the stem in all cases through conical sleeving. The titanium femoral component was cemented in with conventional techniques. The indications for surgery were osteoarthrosis in 228 cases, fracture and nonunion of the hip joint in 35 cases, CDH in 16 and pigmented villonodular synovitis in 12 cases, and various types of hip disorders in 15 cases. The mean follow-up time was 8.2 years (range 1-19 years) for patients with cemented acetabular components and revisions operations were undertaken in 19 cases (12.7%). The mean follow-up time vas 5.7 years (range 1-14 years) for cases with uncemented screw cups, and revision operations were undertaken in 13 cases (9.9%). The mean follow-up time was 2.2 years (range 1-6 years) for cases with pressfit titanium cup without any revision operations. According to the results of survivalship analysis ceramic-onceramic combination is good for hip replacement surgery. The indications for revisions were 16 fractures of acetabular acrtylic cement in the series of cemented acetabular cup, aceptic loosening of the screw cup in 4 cases, late infection in 6 and pigmented villonodular synovitis in 6 cases. The ceramic-on-ceramic combination is good for hip replacement surgery. The cementless pressfit acetabular component with bone transplantation works well in almost all cases and should be used in the first place.
